At Orion Aesthetics, I restore the age-induced volume losses in the face plus increase volume if that is aesthetically desired by the client. Dermal Fillers are of three main types; HA fillers, collagen-stimulating absorbable fillers, and collagen-stimulating non-absorbable fillers.
HA dermal fillers the most commonly used of all the dermal fillers are made up of long chains of hyaluronic acid. Most dermal filler products will consist of HA crosslinked with a chemical such as 1,4-butanedioldiglycidyl ether (BDDE) for Restylane, Belotero and Juvéderm, divinyl sulfone (DVX) for Hylaform, 1,2,7,8-diepoxyoctane (DEO) for Puragen, and suspended in a physiological or phosphate-buffered solution.
The physical properties of a dermal filler are determined by the number of crosslinks by BDDE or DVX, the concentration of the hyaluronic acid and the molecular weight of the hyaluronic acid chain. It is the physical property of the HA filler which decides wherein the face the filler is best placed and most useful. Physical properties of the fillers are also called Rheological properties of the HA gels as mentioned below.
- Elastic modulus (G’): The ability to recover the original shape after shear deformation.
- Viscous modulus (G”): The inability to recover the original shape after shear deformation.
- Complex modulus (G*): The total ability of material to withstand deformation. It is defined as the sum of the elastic modulus (G’) and viscous modulus (G”).
- Cohesivity: The strength of the cross-linking adhesion forces that hold the individual HA units together. Cohesivity is determined by the concentration of HA and the degree of cross-linking. High cohesivity helps the filler maintain vertical projection.
At Orion Aesthetics, I prefer to use Restylane fillers because they have less water absorption later on than other products in the market. This helps me correct the volume more accurately in the first visit and prevents touch-ups or touchdowns later. Because Restylane is very viscous and elastic, it helps me create a natural lip shape and shaping any other part of the face. For the periocular area and lips, I prefer to use Restylane Defyne. If I need to volumize the cheeks, temples, chin or jaws then Restylane Lyft can be used but I prefer Radiesse where I need volume as mentioned below. I also prefer to use Volbella in the lower eyelids or in periocular areas where the skin is very thin, in the lips as well.
Collagen stimulating absorbable fillers are the Radiesse and Sculptra.
Radiesse injectable filler is a sterile, non-pyrogenic, semi-solid, cohesive filler, whose principle component is synthetic calcium hydroxylapatite (30%) suspended in a gel carrier of sterile water for injection, glycerin, and sodium carboxymethylcellulose. Radiesse is made up of the same minerals as the bones or the teeth. It stimulates both collagen and elastin formation in the skin and if placed on the periosteum also leads to new bone formation. While the calcium hydroxylapatite microspheres are completely absorbed in 1 year, the collagen, bone, and elastin formed will stay for 2 years or more thus producing long-lasting volume corrections.
At Orion Aesthetics, I prefer to use Radiesse as my first line filler to improve volume in the medial and lateral maxilla, the zygomatic arch and temporal fossa since it produces a better lifting effect than any HA filler out there. Another advantage of Radiesse as compared to the HA fillers is that it lasts twice as long as the longest-lasting HA fillers in the market.
Sculptra is another useful filler for volume restoration. It is composed of 65micrometers beads of PLLA polymers. The PLLA beads stimulate the fibroblasts to form collagen III. The PLLA beads will ultimately get metabolized into Lactic acid whereas the collagen-induced by them will last for over 2 years. This filler is best used in subdermal space between the dermis and SMAS except for the forehead and temples where it should be used below the temporalis and frontalis muscles. I use this filler whenever a client is not in a hurry to have volume restoration since it takes 6 months to a year for the volume to be restored. Sculptra is the longest-lasting absorbable filler in the market.
At Orion Aesthetics, I do not use the collagen-stimulating non-absorbable fillers like Polymethylmethacrylate.
